A clouded sensorium, also known as an altered sensorium, is a medical condition characterized by the inability to think clearly or concentrate. It is usually synonymous with, or substantially overlapping with, altered level of consciousness. Conclusion: The study revealed a higher incidence of altered mental status among older adults, with a predomi-nance of males. Non-neurological causes were more prevalent than primary neurological causes, although CVA was the leading neurological cause identified. These findings underscore the importance of early recognition, comprehensive assessment, and prompt management of altered sensorium in the ER to improve patient out-comes. METHODS: The study was conducted on 150 patients with altered sensorium of non traumatic origin, over a period of one year at Koppal Institute of Medical Sciences, Koppal after taking consent from patient relatives and ethical committee clearence. All patients of altered sensorium of non-traumatic origin, aged more than 18 years were include in this study. Glasgow coma scale (GCS) is used for primary assessment and also to compare the outcome. The causes of altered sensorium can be varied, some of which are cerebrovascular diseases, alcoholism, cerebral trauma, hypoglycemia, epilepsy, drug intoxication, and neuroinfection. 4–10% of the patients presenting to the emergency department may have altered mental status.
